Beaver Dam Crisis Respite Home for Adults

Beaver Dam, located in Swainton, is a crisis respite group home that provides crisis/respite services to individuals with developmental disabilities. Beaver Dam, opened in 1986, was one of the first programs in the state to provide out of home crisis/respite care. The goal of the program is to provide a safe and caring environment for the individual while they are in living in the home. The program is co-ed and can accommodate a maximum of five individuals at any given time. The home is barrier free and can accommodate one person who is non-ambulatory. To access services for this program the individual must be DDD eligible and a referral must be made through the DDD Case Manager.
